Most insurance are accepted including Medicaid, Anthem Health Keepers Plus and Amerigroup. Please be sure your it is effective. It is the patient’s responsibility to have valid insurance. As participating physicians we will file your claims as a courtesy to you. If your plan requires a co-payment, that fee will be collected at the time of service. If your plan includes a deductible, you are expected to pay us directly. Should we not receive a response from your insurance carrier within 60 days of billing, you will be responsible for your bill. We do not file for secondary insurance.
If we do not participate with your insurance company, payment will be collected at the time of service. You will receive an itemized receipt containing all necessary information enabling you to file for reimbursement.
If you have health insurance coverage, please understand that this is an arrangement between you and your insurance carrier. You are responsible for payment of your bill regardless of the status of your claim.
